Job summary

Cityblock Health is seeking a Care Coordinator to support members with complex health needs in the community. You will work with RN Care Managers to manage caseloads, assess social determinants of health, and develop care plans that advance members toward their health goals.

The role emphasizes strong communication, care coordination experience, and the ability to navigate healthcare systems. A valid driver’s license is required.

Qualifications

• 2+ years in care coordination, community health, or related field.
• Strong critical thinking and judgment with ability to escalate clinical concerns.
• Excellent written, verbal and presentation communication.

Responsibilities

• Collaborate with RN Care Managers to support members with complex medical and behavioral health needs.
• Manage a caseload and address social, behavioral, and environmental barriers to health.
• Develop and implement care plans with members to reach care goals.
• Conduct assessments (PHQ-9, GAD-7, AUDIT, DAST-10) and refer to behavioral health programs as needed.
• Assist with provider appointments, diagnostics, and referrals to community resources.
• Meet members in community settings and document interactions in care platforms.
