Job summary

Alliance Health is seeking a Community Health Worker to assess and assist members with unmet health needs, educate on benefits, and connect them with community resources. The role emphasizes building trust through community engagement and acting as a consultant to the Care Team.

The position is full-time with hybrid work options, requiring occasional onsite meetings and potential regional travel within Wake and Cumberland counties.

Qualifications

• High school diploma or GED required with relevant experience.
• NC Community Health Worker Certification within 12 months of hire.
• Experience with Medicaid/MHSUD benefits preferred or required.
• WRAP/IPS/Trauma-informed care knowledge preferred.

Responsibilities

• Perform SDOH assessments and reassessments.
• Engage members face-to-face and in community settings.
• Connect members to housing, food, transportation, and other resources.
• Assist members with benefit enrollment and care coordination processes.
• Document activities in CM Platform and NCCare360 System.
• Collaborate with Care Team to address barriers and plan care.
