Job summary

• Collaborates with patients and interdisciplinary teams in hospital and community.
• Performs initial assessments to identify focus areas, strengths, and barriers.
• Educates patients on care plans and self-management to improve health.
• Coordinates with community partners and payors to connect patients to resources.
• Documents care coordination and progress in the Medical Respite program records.
• Participates in weekly check-ins with partners and supports discharge planning.
