Job summary

Brown University Health seeks a Medical Respite Community Health Worker to address social determinants of health with individualized support for longitudinal healthcare and social needs. You will assess and coordinate care throughout admission and ensure a warm handoff at discharge for follow-up.

Role requires collaboration with hospital teams and community partners; minimum two years in health care or human services, CHW certification, and strong interpersonal skills are essential.

Qualifications

• Two (2) years of experience providing information, education, intervention and/or referral services to culturally diverse or medically complex patient populations.
• Bilingual English/Spanish preferred.
• Knowledge of health care delivery systems and ability to document in EMR.

Responsibilities

• Collaborate with patients and interdisciplinary teams in hospital and community settings.
• Conduct initial assessments to identify focus areas, strengths, and barriers to care.
• Provide education on care plans and resources; build self-efficacy.
• Coordinate with external stakeholders to arrange needed support.
• Document care coordination and progress notes in the EMR.
• Participate in weekly check-ins with community partners.
