Job summary

Qualifications

• Knowledge of health care delivery systems.
• Analytical skills to evaluate patient needs and plan care.
• Interpersonal skills to interact with patients, families, and providers.
• CHW Certification at hire or within one year.

Responsibilities

• Collaborate with patients and interdisciplinary teams in hospital and community.
• Perform initial assessments to identify focus areas, strengths, and barriers to care.
• Educate patients about care plans and build self-efficacy.
• Coordinate with external stakeholders to connect patients with needed resources.
• Document care coordination and progress notes in the EMR.
• Participate in weekly check-ins with community partners.
• Perform other related duties as assigned.
