Job summary

CVS Health is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to deliver high-quality patient care using evidence-based practices. You will assess needs, administer treatments, and collaborate with interdisciplinary teams to improve outcomes and healthcare delivery.

You will administer medications, monitor patients, document findings in EMR, and guide patients to community resources for self-care and disease management.

Qualifications

• Active and in good standing LVN/LPN license.
• Successful completion of NCLEX-PN exam.
• Knowledge of medical terminology and clinical documentation standards.
• Experience with phlebotomy, vaccines, and point-of-care testing.
• Excellent communication and organizational skills.
• Experience in a clinical setting with diverse populations.
• Ability to manage sensitive information ethically.
• Strong problem-solving and critical thinking skills.
• Epic EMR experience preferred.

Responsibilities

• Administer oral, injectable, intravenous, and topical medications.
• Evaluate complex patient conditions and monitor pain, documenting responses.
• Monitor vital signs and record abnormalities in blood pressure, heart rate, resp rate, and temperature.
• Perform routine assessments to monitor health and well-being.
• Assist patients in accessing healthcare and community resources for meds and disease management.
• Conduct medication reviews for side effects and interactions.
• Develop and administer mental health assessments to screen depression and cognitive function.
• Document patient history in electronic medical records.
• Communicate effectively with patients, families, and health professionals.
