Job summary

Reema Health is seeking a Community Health Navigator to build relationships with members and guide them toward greater engagement in their health. You will travel within a 30-mile community area, maintain a caseload of 80-120 members, and document care plans using Reema's technology platform.

You will work Monday–Friday with occasional evenings; a valid vehicle and insurance are required. This is a fast-paced startup role with evolving responsibilities and opportunities to grow the program and

Qualifications

• Able to troubleshoot and adapt to new tools on mobile and desktop
• Excellent verbal and written communication
• Strong problem-solving and empathy in community settings
• Self-starter who stays productive on a remote caseload
• Comfort working with a multidisciplinary team and diverse community members

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model
• Build and manage a caseload (80-120 members) and cover for others as needed
• Assess member needs, including social determinants of health, to build care plans
• Coordinate referrals and schedule appointments; follow through to completion
• Independently organize workload to hit monthly goals and daily metrics
• Engage members through multiple channels and connect them to resources
• Maintain confidentiality and document all work in the Reema App
