Job summary

City of New York – NYC Health Department seeks a Community Coordinator to support the Creating Health Schools and Communities (CHSC) grant. You will coordinate CHSC strategies, engage schools and community partners, and track program data to meet deliverables.

The role involves collaboration with schools, residents, and city/state agencies to advance physical activity, active transportation, and built environment initiatives in East Brooklyn, with occasional evening/weekend commitments.

Qualifications

• Bachelor's degree and two years of community work or related activities.
• Experience coordinating programs, coalitions, or multi-partner initiatives.
• Ability to interpret and communicate public health information to diverse audiences.

Responsibilities

• Coordinate CHSC grant strategies, activities, and deliverables.
• Lead Complete Streets, active transportation, built environment, and physical activity initiatives.
• Convene coalition and stakeholder meetings related to school wellness and community health.
• Develop and maintain relationships with schools, community organizations, residents, and government agencies.
• Support school-based CHSC strategies, including wellness councils and policy changes.
• Provide technical assistance and capacity-building to partners.
• Track program data and monitor grant deliverables and progress.
• Assist in preparing CHSC reports and presentations.
