Job summary

The Center for Health Equity & Community Wellness (CHECW) within the DEPT OF HEALTH/MENTAL HYGIENE seeks a Program Coordinator to advance the Creating Health Schools and Communities (CHSC) grant funded by the New York State Department of Health.

The Program Coordinator will coordinate CHSC activities to improve access to physical activity, active transportation, and school/community wellness in East Brooklyn.

Responsibilities

• Coordinate CHSC grant strategies, activities, work plan objectives and deliverables.
• Lead CHSC Complete Streets, active transportation, and built environment initiatives.
• Convene CHSC coalition and stakeholder meetings on transportation, activity, school wellness.
• Develop relationships with schools, community organizations, residents, and agencies.
• Support school-based CHSC strategies, including wellness councils and activities.
• Collect and review CHSC program data and assist with reporting.
