Job summary

The NYC Department of Health and Mental Hygiene is seeking a Community Coordinator to lead outreach and health education efforts within the Bureau of Communicable Disease. The role focuses on applying cultural competence to engage diverse communities and support equitable health access across New York City.

Qualifications

• A baccalaureate degree from an accredited college with two years of related community work experience.

Responsibilities

• Manage health education priorities within the bureau to plan, coordinate, and monitor community outreach initiatives related to infectious diseases.
• Organize and facilitate outreach initiatives within the community to address disparities in health access and information.
• Assist in the monitoring and maintaining of critical health educational materials, inventory control and ordering processes for health education materials and incentive items to support outreach and education programs.
• Update and develop outreach and health education materials such as slide decks, training activities, handouts, lesson plans and resources to support community outreach and ensure they incorporate a racial equity lens.
• Develop and maintain collaborative relationships with colleagues within DOHMH, and external partners, to align and enhance health education initiatives.
• Track and evaluate outreach and education data to prepare comprehensive reports highlighting the impact of community outreach programs on health equity and infectious disease control.
• Attend and present at meetings with internal and external stakeholders to support Community Engagement and Health Education activities.
• Support seasonal staff and interns during peak outbreak activities.
• Participate in the Incident Command System to support emergency response activities as needed.
• Work on special projects as needed.
