Job summary

Public Health Solutions, Inc. seeks a Community Action Coordinator to lead the Queens Healthy Start Community Consortium, drive a shared agenda, and coordinate partnerships to address maternal and child health disparities in South Queens.

Under supervision of the Program Manager, this role will gather and analyze data to identify health needs, develop an annual strategic plan, and guide stakeholders to implement equitable community actions and policy outreach.

Qualifications

• Minimum Associate’s degree or equivalent.
• 1+ years of experience in community organizing, strategic planning, and supporting cross-sector partnerships.
• Able to travel to various partner sites across South Queens including Far Rockaway.
• Experience in public health and community development in urban, underserved neighborhoods
• Bi-lingual preferred (Spanish or Haitian Creole), but monolingual English speakers are also encouraged to apply.

Responsibilities

• Lead/facilitate bi-monthly QATF meetings, virtually or in person.
• Facilitate the bi-monthly executive team meetings in collaboration with the QATF executive team members.
• Conduct continuous targeted outreach to community partners and local community groups, agencies, and organizations to identify partners and build a cross-sector Consortium.
• Work with partners to identify, recruit, and retain community residents to participate in the Consortium.
• Coordinate training for Consortium members in skills such as community-based participatory research, data analysis, and asset mapping and on topics such as toxic stress, trauma-informed care, and health insurance enrollment and others QATF has selected.
• Provide guidance and lead QATF in selecting a common agenda, shared measures, and mutually reinforcing activities.
• Represent QHS and QATF at outreach events and stakeholder meetings.
• Manage the QHS social media account by designing content, scheduling posts, and engaging with followers.
• Work with PHS Communications team to develop the QATF monthly newsletter and awareness month email campaigns.
• Plan and organize large-scale events such as awareness events, focus groups, town halls, health, and educational sessions, and outreach events.
• Work with PHS Evaluation team to design and implement evaluation activities for the Consortium.
• Represent Queens Healthy Start with elected officials.
• Draft reports regarding grant activities and program evaluation and outcomes.
• Other responsibilities as assigned by the Queens Healthy Start Program Manager.
