Job summary

Community Health Centers, in the Medical/Health Education department, seeks a Health Educator to lead health communication and patient education efforts. Under supervision, you will teach wellness behaviors and develop learning experiences that empower individuals to make informed health decisions.

The role supports disease prevention and management, collaborates with care teams, and coordinates community resources and health fairs.

Qualifications

Master's degree in health education or related field.
Minimum 5 years of health educator experience preferred.
Bilingual English/Spanish preferred; reads/writes/speaks well in both languages.
Experience with Electronic Health Records (EHR) required.
Strong communication and community outreach skills.

Responsibilities

Lead planning and evaluation of health education for patients and community.
Design health education policies and train clinical staff.
Assess health literacy and adapt programs to patient needs.
Provide health education on topics such as HIV/AIDS, diabetes, pregnancy testing, and smoking cessation.
Organize health fairs and conduct classes, including evenings/weekends.
Document goals and outcomes in the Electronic Health Record.
Develop relationships with community partners to support patients.
