Job summary

Catholic Health seeks a Community Health Educator to plan, coordinate, and facilitate diverse health education and chronic disease prevention programs. Based at the DeMatteis Center and representing St.

Francis Hospital, this role focuses on managing chronic conditions and ensuring accreditation standards. You will develop nutrition and prevention programming, organize health fairs, supervise smoking cessation and falls prevention classes, and collaborate with the Pharmacy Department on patient

Qualifications

Excellent oral and written communication skills required.
Proven experience tracking registration, managing extensive course handouts, coordinating speaker schedules, and utilizing virtual meeting platforms (Zoom).
Comfort working with independent contractors, clinical specialists, and hospital committees.

Responsibilities

Create and conduct educational programming centered on nutrition, hygiene, smoking cessation, and prevention of chronic conditions (diabetes, heart disease).
Plan, organize, and execute community health fairs with vital screenings (blood pressure, cholesterol).
Lead stroke support and caregiver sessions monthly; coordinate speakers and join accreditation committees.
Lead Diabetes Prevention program groups with a Diabetes Educator.
Represent SFH at local senior centers and libraries to promote education programs.
Participate in DeMatteis Center Nursing Shared Governance Council and attend monthly meetings.
