Job summary

The Community Health Centers of the Central Coast is seeking a Health Educator to lead health promotion and disease prevention efforts under supervision of the Health Education Coordinator and Nursing Administration.

The HE will design and deliver education programs, assess health literacy, and coordinate with care teams to support patient self-management and community health initiatives. Bilingual English/Spanish preferred; experience with EHR and health education required.

Qualifications

• Master's degree in public health education or health promotion.
• Minimum 5 years of health education experience preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Lead planning, implementation, and evaluation of primary prevention and risk reduction for patients and community.
• Assist in designing health education policies and training clinical staff.
• Assess health literacy and adapt programs; document in electronic records.
• Provide health education on topics like HIV/AIDS, diabetes, and preventive care.
• Organize health fairs and teach evening or weekend classes as needed.
• Coordinate with care teams to ensure appropriate services for patient goals.
• Maintain HIPAA compliance and confidentiality; participate in quality improvement.
