Job summary

Community Health Centers of the Central Coast, Inc. seeks a Health Educator to lead health promotion and disease prevention within the Medical/Health Education department.

The role focuses on planning, implementing, and evaluating prevention strategies and educating patients and communities to improve wellness. Under supervision, the HE will coordinate programs, document goals in the EHR, and collaborate with community partners to meet patient needs.

Qualifications

• Master's degree in public health education, community health education, school health education, or health promotion.
• Minimum 5 years of experience as a health educator preferred.
• Bilingual English/Spanish is preferred; read, write and speak skills desirable.
• Knowledge of HIPAA, patient confidentiality, and health education ethics.
• Ability to work in evenings and some weekends to support classes and events.

Responsibilities

• Lead planning, implementation, and evaluation of primary prevention and risk reduction for patients and community.
• Assist in design and implementation of health education policies and staff training.
• Assess patient health literacy and adapt programs to meet needs; document in EHR.
• Provide general health education topics (HIV/AIDS, asthma, diabetes, pregnancy, etc.).
• Set up health fairs and teach classes as needed, including evenings/weekends.
• Document self-management goals for chronic diseases and coordinate with care team.
• Develop community partner relationships and ensure safety protocols are followed.
