Luminis Health is seeking a proactive professional to lead and support strategic planning, implementation, and evaluation of community-based health programs. The role partners with leaders to address health disparities and to advance outreach and population health initiatives in diverse communities.

Qualificações

• Bachelor's degree in community health, public health, or related field.
• Master’s degree preferred.
• Five (5) years of experience.
• CHES certification or a Masters degree in community health, public health, or related field may substitute for one (1) year of the required experience.

Responsabilidades

• Leads strategies that measurably improve health and wellbeing in underserved communities.
• Coordinates staffing and resources for community education programs, events and screenings; acts as onsite coordinator.
• Updates, evaluates, and develops prevention and wellness programs based on best practices and current research.
• Identifies community benefit program opportunities and develops innovative approaches to population health improvement.
• Develops information, educational marketing and outreach materials for community education.
• Maintains community outreach calendar and dashboard reports.
• Provides direct education on a wide variety of community and public health topics.
• Conducts research, data collection, analysis and writing on community benefit programs and health needs assessments.
• Collects Community Benefit Report data with the Director; includes financial and qualitative data.
• Provides education and training to department leaders on community benefit principles and use of software and record-keeping.
• Identifies cultural patterns and health disparities; develops and evaluates community health programs.
