Job summary

Luminis Health in Annapolis, MD seeks a Community Health Program Manager to lead planning, implementation and evaluation of outreach programs addressing health disparities. You will coordinate staffing, develop prevention initiatives, and collaborate with partners to improve population health in diverse communities.

Excellent communication, project management, and cultural competence are essential. Travel to multiple sites within Anne Arundel County may be required, and evenings/weekends

Qualifications

• Bachelor's degree in community health, public health, or related field; Master’s preferred.
• Five (5) years of experience in related roles.
• CHES certification or a Master’s degree may substitute for one year of experience.
• Must travel within and outside Anne Arundel County and attend evening/weekend meetings, events, and programs.
• Must maintain a valid Maryland driver’s license.

Responsibilities

• Leads strategies to measurably improve health in underserved communities and coordinates staffing for education programs.
• Develops outreach materials and maintains outreach calendar; provides direct education on health topics.
• Conducts research and data collection on community benefit programs and population health; analyzes data with department leadership.
• Collaborates with external partners and represents the organization on coalitions and planning groups; recruits community members for engagement.
