University Clinical Health, Inc. is seeking a Health Coach to provide frontline wellness leadership, motivational interviewing, and patient-centered coaching within a busy Health Hub.

The role emphasizes documenting care in Epic EHR, scheduling, and coordinating with clinicians while promoting chronic disease self-management and healthy lifestyles. Some evening and weekend events may be required.

Formación

• Requires training and certification in Motivational Interviewing and Health Coaching within the first 45 days of employment.
• High School Diploma or GED required.
• Strong interpersonal and relationship-building skills.
• Experience with diverse populations.
• Excellent verbal and written communication.
• Proficiency with electronic health records and healthcare technology.
• Experience providing care for individuals with chronic conditions.
• Ability to work independently and collaboratively.
• Valid driver’s license.

Responsabilidades

• Deliver front desk and administrative support to Health Hub operations, scheduling, and documentation.
• Conduct health screenings and health coaching sessions, including motivational interviewing.
• Maintain HIPAA-compliant records in Epic EHR and ensure accurate patient management.
• Coordinate outreach activities and community engagement to promote the Health Hub.
• Support care navigation and referrals to medical, behavioral health, and social services.
• Assist with calendar management, meetings, and supply management.
• Provide receptionist duties and phone coverage as part of daily operations.
