Expect active engagement with clinicians, schedule coordination, and documentation in the Epic EHR to ensure HIPAA-compliant patient care. The role focuses on improving patient experience, coordinating care navigation, and supporting community outreach efforts

Qualificações

• Must complete Motivational Interviewing and Health Coaching Certification within the first 45 days of employment.
• High School Diploma or GED required.
• Proficiency with electronic health records and healthcare technology.

Responsabilidades

• Provide administrative support to Health Hub operations and staff.
• Schedule and coordinate appointments, meetings, and calendar activities.
• Perform health screenings and deliver health coaching sessions.
• Maintain HIPAA-compliant documentation in the EHR and patient records.
• Coordinate community outreach and distribution of health education materials.
