Responsibilities

• Provide health coaching and consultation services within state licensure requirements.
• Collaborate with patients to set goals and promote healthy lifestyle choices.
• Coordinate with primary care staff and providers to obtain current health information.
• Review data sources to identify comorbidities, hospitalizations, and plan interventions.
• Assist in care coordination, including test/referral results and communication with community health orgs.
• Proactively address patient needs through population health management.
• Create and manage patient registries for chronic disease and preventive care.
• Document all activities per Valley Health Policy Manual.
• Support operations in collecting data for NCQA accreditation and meaningful use.
• Other duties as assigned.
