A federally qualified health center in Albuquerque is seeking a Case Manager to support individuals and families through case management, care coordination and connection to community-based services.
The organization provides healthcare and support services to underserved populations, including individuals experiencing homelessness and patients with complex medical, behavioral health and social-service needs. The Case Manager will work directly with patients and interdisciplinary teams to identify barriers to care, coordinate services and connect individuals with appropriate internal and community resources.
Experience in community outreach, housing programs, public health, harm services, HIV services, peer support or community-based case management is especially relevant to the work.
