The Case Manager will advocate for patients, navigate the healthcare system, and partner with medical, behavioral health, and social services teams to improve health outcomes.

Qualifikationen

• Certified Peer Support Worker (CPSW) preferred or Bachelor’s degree in Social Work, Public Health, Psychology, or related field.
• 2+ years of case management or care coordination experience, ideally in FQHC or community health setting.
• Knowledge of community resources and social service programs in New Mexico is a plus.

Aufgaben

• Provide case management services including care coordination, referrals, and resource navigation
• Conduct patient assessments to identify medical, behavioral, and social support needs
• Connect patients with community resources such as housing, transportation, food programs, and behavioral health support
• Collaborate with clinical and behavioral health teams to develop and implement individualized care plans
• Maintain accurate documentation in the EHR and track patient progress
• Support patient engagement, education, and follow-up to improve health outcomes and continuity of care
