Job summary

Leoforce in Albuquerque, NM seeks a Case Manager – Community Health (FQHC) to support patients by coordinating care, connecting to resources, and improving health outcomes.

Onsite Monday–Friday, 8:00am–5:00pm, you will collaborate with medical, behavioral health and social services teams, advocate for patients, and address social determinants of health such as housing, transportation, and food access.

Qualifications

• Bachelor’s degree in Social Work, Public Health, Psychology, or related field preferred.
• CPSW certification preferred when available.
• Experience with case management or care coordination in FQHC/context preferred.
• Experience working with underserved populations is strongly preferred.

Responsibilities

• Provide case management services including care coordination, referrals, and resource navigation.
• Conduct patient assessments to identify medical, behavioral health, and social support needs.
• Connect patients with community resources such as housing, transportation, food programs, and behavioral health support.
• Collaborate with clinical and behavioral health teams to develop individualized care plans.
• Maintain accurate documentation in the EHR and track patient progress.
• Support patient engagement, education, and follow-up to improve health outcomes and continuity of care.
