Qualifications

• Active NC LPN or RN license or degree in human services or healthcare management field.
• Experience in direct patient care in outpatient or inpatient settings.
• Bilingual (Spanish) is a plus; able to serve in translating capacity.

Responsibilities

• Participate actively in team-based care.
• Provide case management services and discharge planning for uninsured/underinsured patients.
• Complete initial patient/family assessments and develop problem lists.
• Identify and prioritize at-risk patient populations.
• Perform occasional home visits and follow-up as indicated.
• Collaborate with patients, physicians, community health centers and other providers to develop a comprehensive care plan.
• Communicate care plans clearly to patients and colleagues, verbally and in writing.
