Job summary

Harris Health System in Houston, TX seeks a Community Health Worker (CHW) to join an interdisciplinary care team. The CHW will assess patients’ needs across economic and social determinants, partner to develop a team-based action plan, and support ongoing engagement to improve overall health outcomes.

The role may include eligibility screening, health insurance enrollment, prevention screening, and coordinating special projects, with a focus on accessibility and culturally competent care.

Qualifications

• Bachelor's degree in health services, public health, sociology, psychology or related field required or equivalent.
• 1 year of work experience with patient navigation, care management, or direct patient service in community health or healthcare.
• Or 5 years of work experience in patient navigation or care management in community health or social/healthcare settings.
• Texas DSHS CHW certification within first year of employment.

Responsibilities

• Assess patients' needs across economic, social, physical and mental health aspects.
• Coordinate a team-based action plan and establish patient priorities with goals.
• Serve as point person for a panel of patients and provide accessible support.
