Job summary

CAMBA seeks a Community Health Worker in Brooklyn to outreach to high-need women and infants and connect them to CAMBA services and community resources.

You will conduct intake and assessments, facilitate planned educational presentations, and maintain participant files while managing a caseload with culturally diverse families. Bilingual Spanish and English is highly preferred to support the community served.

Qualifications

• High School diploma or GED with 4 years of applicable experience or equivalent experience.
• Experience working with or providing services to children and their families is preferred.
• Bi-lingual Spanish and English is highly preferred.

Responsibilities

• Conduct outreach to identify high-need women and infants at community venues and events.
• Plan and facilitate educational presentations for participants and providers.
• Assist participants in completing CAMBA intake applications and consent forms.
• Maintain participant files and manage caseloads across the life cycle.
• Advocate for participants with referral sources regarding education, healthcare and housing.
