Job summary

CAMBA INC in Brooklyn, NY seeks a Community Health Worker to interview and evaluate applicants for services, create service plans, and help participants implement those plans. You will also advocate for participants and connect them with needed resources.

Responsibilities include outreach to recruit participants, education on childbearing, reproductive health, parenting, and child development, and serving as liaison with referral sources. Some evenings/weekends may be required.

Qualifications

• High School Diploma or GED with 4 years of applicable experience.
• Bi-lingual Haitian Creole/English or Spanish/English preferred.

Responsibilities

• Interview and evaluate applicants for services, formulate service plans and goals, and aid participants to implement service plans.
• Work with participants to overcome barriers/goals, assist participants in advocating for themselves and in moving toward self-sufficiency.
• Provide education on topics related to childbearing, reproductive health, child safety, parenting and infant/child development.
• Act as liaison and advocate for participants with referral sources regarding progress related to education, healthcare, housing, etc.
• Locate and connect participants to appropriate community resources.
• Conduct street outreach to recruit participants from local businesses, schools, hospitals, clinics, etc.
• Plan and utilize creative outreach strategy events to motivate participants to use CHW program services.
