Job summary
Your Health in Canton region seeks a dedicated Community Health Worker to visit patients in homes and ALFs, facilitating care with providers and delivering medications and DME. You will coordinate telehealth and RPM activities to ensure timely, safe care across Georgia and South Carolina, and travel daily with reliable transportation.
The role is full-time with 12-hour shifts (7:00 AM–7:00 PM). Prior experience in home health, public health, or DME logistics is highly valued, as is a valid
Qualifications
- Licensed healthcare professional preferred (Nurse/Paramedic/MA).
- Phlebotomy experience preferred; training available if needed.
- Experience in public health or home health is a plus.
- Experience with DME delivery or logistics is beneficial.
- Comfort with RPM, telemedicine, and EMR systems.
- Strong organization, communication, and time-management skills.
Responsibilities
- Coordinate and facilitate visits with care team, providers, and facilities.
- Deliver medications and DME to patients' homes and ALFs; reconcile meds.
- Coordinate telehealth visits with pharmacists, specialists, and PCPs.
- Instruct patients on RPM equipment; provide ongoing support and troubleshooting.
- Assist with medication administration and ensure accurate lab specimen handling.
- Maintain clear communication with patients and caregivers; document interactions.
