Job summary

Your Health in Aiken, SC is seeking a dedicated Community Health Worker to support patients in homes and care facilities. You will coordinate visits with care teams, deliver medications and DME, and facilitate telehealth services to ensure timely access to care.

Ideal candidates have healthcare certification and comfort with RPM/EMR tools, with reliable transportation for daily travel. This is a full-time role with 12-hour shifts (7:00 AM–7:00 PM) across designated service areas.

Qualifications

• Certified Medical Assistant, Paramedic, or licensed nurse preferred with good standing.
• Experience with home health or public health settings is a plus.
• Proficient with technology for remote patient monitoring and EMR systems.

Responsibilities

• Visit patients in homes and ALFs to coordinate care with providers.
• Deliver medications and DME, ensuring correct instructions and safety.
• Coordinate telehealth visits between patients and pharmacists or providers.
• Set up RPM equipment training and provide ongoing support.
• Assist with venipuncture for lab testing and sample handling.
• Maintain accurate records and communicate effectively with care teams.
• Travel daily to service areas and manage logistics for visits.
• Ensure continuity of care in collaboration with care teams.
