Job summary

Your Health Organization is hiring a Community Health Worker to serve Savannah-area patients, with home and facility visits, medication delivery, and DME support. Travel required daily; 12-hour shifts (7:00 AM–7:00 PM). This role partners with care teams to ensure coordinated, accessible patient care across facilities, homes, and clinics.

The CHW will engage with telehealth, RPM training, and medication reconciliation while supporting patients and families in managing care plans.

Qualifications

• Certified/licensed professional in healthcare; valid license in good standing.
• Phlebotomy experience preferred; willing to train if needed.
• Experience in public or home health is a plus.
• Able to operate DME delivery/logistics and use remote monitoring tech.

Responsibilities

• Visit patients at homes or Assisted Living Facilities to coordinate care.
• Deliver medications and DME, reconcile medications with patient lists.
• Coordinate telehealth visits between patients, pharmacists, and providers.
• Assist with RPM setup and patient training; provide ongoing support.
