Job summary

Molina Healthcare is seeking a Community Health Worker in Cincinnati to support community-based member advocacy and help vulnerable members manage health care needs. You will serve as a local resource, engage with members in homes, shelters, and provider offices, and contribute to quality and cost-effective care.

Requirements include at least 1 year of health care experience, a valid driver's license, and strong communication and organizational skills.

Qualifications

• 1+ year healthcare experience, preferably with underserved or special needs populations.
• CHW certification may be required in some states.
• Valid and unrestricted driver's license with reliable transportation.
• Ability to multi-task while speaking with members.
• Excellent customer service, organization, and time management.
• Willingness to learn other programs and software systems.

Responsibilities

• Advocate for members and help manage healthcare needs in the community.
• Meet with members in homes, shelters, provider offices, and other settings.
• Help navigate health plan benefits and schedule appointments.
• Arrange transportation and follow up on missed visits.
• Assist with access to social services and community resources.
• Reach out to locate disconnected members with special needs.
• Conduct outreach and potential in-person or telephonic contacts.
• Support Medicaid eligibility maintenance and related financial resources.
• Travel locally 50-80% as required by contract.
