Qualifications

• Preferred CHW certification completed or in progress.
• Additional education in healthcare, human services, social work, or related field is preferred.
• Experience in a physician health care office or similar setting is preferred.
• Clear oral expression, listening, writing, analytical reasoning, and use of EMR systems.

Responsabilités

• Advocate for clients and help reduce barriers to receiving appropriate care.
• Connect clients with healthcare, social services, and community resources.
• Provide health education and information in a culturally appropriate manner.
• Assist clients with applications for healthcare, food, housing, transportation, and other assistance programs.
