Job summary

Maryland Care Management, Inc. is seeking a patient-focused care coordinator to work closely with medical teams and social services. The role involves short-term care coordination and connecting clients to resources in both clinical and home settings.

You will educate clients, document interactions, and help navigate barriers to care, including transportation and housing. Occasional travel is required for in-home visits and community outreach.

Qualifications

• High school diploma or GED.
• 2 years’ experience in serving the Medicaid population, preferably in the MD/DC area.
• Community-based work experience and/or CSW/CPSW certification preferred.

Responsibilities

• Communicates program purposes and impact to clients and helps develop health and social plans.
• Documents client encounters and maintains electronic files; submits monthly reports.
• Educates clients on appropriate use of Emergency Services and self-care of chronic conditions.
• Assists clients in accessing medical homes and social services; helps overcome barriers to care.
• Provides support during medical visits and ensures referrals are conveyed.
• Facilitates access to community resources (housing, food, clothing, parenting, mental health services).
• Coordinates services between providers and clients and tracks care plan compliance.
• Builds relationships with clients and providers to reduce barriers across cultures and socio-economic backgrounds.
