Job summary

Maryland Care Management, Inc. (MCMI) is seeking a Care Coordinator to support the care management team by providing short-term care coordination and linking members to resources. You will collaborate with members, providers, and the CM team to educate and coordinate services, help members access a medical home, and address barriers to care.

This role emphasizes relationship-building, data collection for HRAs, and maintaining comprehensive member records to improve outcomes.

Qualifications

• High school diploma or equivalent.
• Two or more years of experience serving the Medicaid population in MD/DC preferred.
• Strong interpersonal, communication, and customer service skills.
• Ability to work with diverse populations.
• Proficiency with computers and Microsoft Office.
• Understanding of remote work tools (Teams, Zoom, SharePoint).

Responsibilities

• Provide administrative support to CM team members.
• Collect data for Health Risk Assessments (HRA).
• Screen for eligibility and benefits.
• Identify members without a PCP and refer to Member Services.
• Coordinate care transitions with PCPs and providers.
• Produce and mail routine CM letters and materials.
• Maintain electronic member records and monthly reports.
• Educate members and arrange PCP visits.
• Assist members in accessing health-related services.
• Support coordination of community resources and address barriers to care.
