Job summary

Maryland Care Management, Inc. (MCMI) is seeking a dedicated Care Coordinator to serve Maryland Physician Care clients across clinical and community settings, including home visits.

Under indirect supervision, you will help identify barriers, coordinate care, connect clients with social services, and educate to improve health and wellbeing.

The role requires 2-3 in-person days in Baltimore, MD, and offers competitive pay along with medical, dental, vision, 401k match, and PTO.

Qualifications

• High school diploma or GED required.
• Two years’ experience serving Medicaid populations, preferably in the MD/DC area.
• CSW or CPSW certification preferred.
• Strong interpersonal and communication skills.

Responsibilities

• Communicate program purposes and help clients identify socio-economic barriers affecting health.
• Document client encounters and maintain electronic files; prepare monthly reports.
• Educate clients on appropriate emergency room use and self-management of chronic conditions.
• Assist clients in obtaining a medical home and accessing social services.
• Coordinate services between providers and clients and monitor care plan adherence.
• Build relationships with clients, providers, and community staff to reduce barriers.
