Job summary

Maryland Care Management, Inc. (MCMI) is seeking a Care Coordinator to work with the CM team, helping members obtain care, education, and access to services.

You will collect data for risk assessments, screen for eligibility, and coordinate with PCPs and other providers. The role emphasizes support for members' health and well-being, collaboration with a diverse team, and the use of remote tools to communicate and coordinate effectively.

Qualifications

• High school diploma or equivalent required.
• 2+ years experience serving Medicaid population in MD/DC area preferred.
• Strong interpersonal and communication skills are essential.

Responsibilities

• Provide administrative support to CM team members.
• Collect data for Health Risk Assessments.
• Screen for eligibility and benefits.
• Identify members without PCP and refer to Member Services.
• Coordinate care transitions with providers and care managers.
• Produce and mail routine CM letters and educational materials.
• Maintain electronic member records and monthly reports.
• Assist members in accessing health services and community resources.
• Facilitate communication between providers and members.
