Job summary

DAP Health seeks a Care Coordinator Specialist to educate communities about affordable public programs and assist with enrollment and ongoing eligibility. You will conduct outreach at events, help with new patient registrations, and provide case management to ensure access to preventive care and benefits across San Diego and Riverside counties.

The role requires strong communication in English and Spanish, attention to detail, and the ability to navigate multiple systems and documentation.

Qualifications

• Current CEC certification.
• 1-2 years of experience in health care or social service preferred.
• Bilingual in Spanish/English preferred.

Responsibilities

• Educate communities about public programs (Medi-Cal, Covered California, Sliding Fee, etc.).
• Assist enrollment and follow-up; resolve eligibility barriers with agencies.
• Complete patient registration and coordinate referrals and appointments.
• Provide extensive follow-up/case management to ensure utilization of services and retention of benefits.
