Job summary

DAP Health seeks an RN Case Manager to advocate and coordinate care for at‑risk patients, including those recently discharged or with chronic diseases. You will assess needs, arrange services, and collaborate with the care team to improve outcomes and access to care.

You will document thoroughly, work well under pressure, and engage with diverse populations in a hybrid clinic environment. Bilingual Spanish/English preferred.

Qualifications

• RN degree and licensure.
• Minimum 2 years in case management or related field.
• Strong knowledge of management principles and practices.

Responsibilities

• Conduct thorough needs assessments based on referral information, including medical history and social factors.
• Arrange and coordinate services such as transportation, in-home support, and medications.
• Assist with paperwork to obtain services and maintain accurate documentation.
• Research and resolve complex cases by leveraging available resources.
• Advocate between agencies and clients to ensure timely interventions and informed decisions.
• Communicate interventions and progress with the health care team and conduct follow‑ups.
