Job summary

DAP Health Sunrise - Palm Springs, CA is seeking a Wellness Navigator to join our care team, focusing on care coordination, linkage to services, and health equity. You will work across clinics to reduce barriers to care and support patients throughout their healthcare journey.

The role requires strong communication, ability to work independently within a multidisciplinary team, and experience in clinical or community health settings.

Qualifications

• Three years’ experience in a clinical or community health setting.
• Care coordination experience and health system navigation knowledge.
• Familiarity with EHR/patient tracking systems and state programs.

Responsibilities

• Develop expertise in Epic population management to track high‑risk patients and engagement.
• Document patient encounters accurately using appropriate templates and workflows.
• Maintain a caseload, track attendance, identify barriers, and assist with scheduling/follow-up.
• Provide outreach to initiate and complete care in a timely manner.
• Collaborate with staff to address barriers to care and coordinate with external providers.
• Participate in multidisciplinary case conferences to support care planning and continuity.
• Ensure care documentation is complete and accessible to care team members.
• Support quality assurance, improvement, and special projects related to patient care.
