Virgin Islands is seeking a Family Support Specialist II in the VI Office – St Croix to provide comprehensive case management, intake assessments, and coordinated access to community services for families, seniors, and disabled residents under the Choice Neighborhoods program. You will work with a team of social workers and relocation and employment specialists, maintain privacy of client information, and document progress in LEARN data system.

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology or related field.
• Minimum of three years of work experience as a case manager or similar role.
• Commitment to confidentiality and professional handling of privileged information.

Responsibilities

• Conduct intake and assessments for enrollment into the case management program.
• Conduct home visits, office visits, and community site visits to collect information.
• Develop and monitor Development Plans using a multidisciplinary approach.
• Assist residents in navigating social services and public agencies.
• Identify gaps in services and advocate for residents on caseload.
• Maintain privacy and confidentiality of all household information.
• Enter and track resident data in LEARN to document progress in real time.
• Attend community meetings with partners and social service entities.
• Participate in Urban Strategies program meetings related to the project.
• Occasionally work nights/weekends and travel as required.
• Monitor budgets, prepare reports, and submit billing documentation as needed.
• Perform miscellaneous duties as assigned.
