Job summary

Urban Strategies Inc. seeks a Family Support Specialist II to deliver comprehensive case management and coordinate with community partners. You will support residents and families to promote self-sufficiency and access a broad range of services, while maintaining confidentiality and working with a multidisciplinary team.

You will conduct assessments, enter progress into LEARN, and participate in program meetings. Some evening, weekend work and travel may be required in service of the project.

Qualifications

• Bachelor's degree in social services or related field.
• Minimum three years of direct case management experience with low-income households.
• Strong commitment to confidentiality and ethical practice.

Responsibilities

• Conduct intake and assessments for enrollment into the case management program.
• Perform home, office, and community visits to collect information.
• Develop and monitor Development Plans with a multidisciplinary team.
• Navigate social service systems and public agencies for residents.
• Identify service gaps and advocate to coordinate final service delivery.
• Enter and track resident progress in LEARN in real time.
• Attend community supportive services meetings with partners.
• Participate in program meetings and related events.
• Occasionally work nights/weekends and travel as required.
• Perform administrative duties including budgets, reports, and billing documentation.
