Job summary

Friends Aware, Inc. seeks a Community Development Specialist to provide direct support to program participants and assist with personalized plans. The role involves personal care, documentation, and collaborative planning.

The position includes coordinating activities, supervising participants, and transporting them to on-site and community events. Must be able to work in a team, maintain confidentiality, and follow agency policies.

Qualifications

• Certified Medication Technician (CMT) certification required or able to obtain.
• DOT certification or ability to obtain DOT certification.
• Must possess typing skills sufficient to be productive.
• Valid driver’s license with a minimum of 3 years of driving experience and an agency approved driving record.

Responsibilities

• Provides direct support to program participants in accordance with individual plans.
• Assists with personal care (toileting, dining, bathing) as needed.
• Completes documentation of responses and daily activities.
• Attends PCP meetings and reviews consumer progress.
• Plans and facilitates individual and group activities on-site and in the community.
• Transports consumers to activities and outings as needed.
• Maintains safety and confidentiality and adheres to policies and procedures.
