Job summary

You will help participants engage in meaningful activities, provide supports per their plans, and ensure safety during all program events. The role includes assisting with personal care, promoting communication of preferences, and transporting participants to activities within the community.

Qualifications

• Must have the ability to read and interpret participant program plans and procedure manuals.
• Ability to write routine reports and present information clearly to individuals and small groups.
• Basic computer operation and data entry skills are required for participant tracking and incident reporting.

Responsibilities

• Assist participants to develop and engage in meaningful activities.
• Provide supports identified in participants’ approved plans.
• Ensure safety and well-being for participants during all program activities.
• Provide personal care to participants, including hygiene, meals, and minor medical issues.
• Encourage participants to communicate and advocate their preferences.
• Provide transportation for participants to Community Day Services locations and community activities.
