Job summary

Heart to Heart Hospice seeks a Community Education Representative to drive hospice referrals by cultivating relationships with physicians, hospitals, and senior living communities. This sales-focused role develops territory plans and promotes Heart to Heart Hospice House and related services to referral sources.

You will work with hospice-eligible patients and families during admission, manage accounts with CRM tools, build public relations efforts to raise brand awareness, and ensure compliance

Qualifications

• 1–3 years of progressive sales experience in hospice/home health/healthcare sales preferred.
• Knowledge of business management and Medicare Hospice Benefit.
• Excellent time management and organizational skills; ability to work autonomously.

Responsibilities

• Call on physicians, area hospitals, SNFs, assisted living facilities, and other senior living settings to promote hospice referrals and Heart to Heart Hospice House.
• Work with hospice-eligible patients and their loved ones during admission in collaboration with Hospice House staff.
• Develop and implement territory business plans and manage sales time/resources to meet goals.
• Establish and maintain ongoing public relations to increase brand awareness and community understanding of the organization and services.
• Utilize CRM and other tools to effectively manage accounts and performance.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and company policies.

What You Must Have:

• One to three years of progressive sales experience in hospice sales, home health, or other healthcare sales strongly preferred.
• Knowledge of business management and a strong knowledge of the Medicare Hospice Benefit
• Excellent time management and organizational skills, with the ability to meet deadlines and work autonomously
• Valid driver’s license
• Reliable transportationand provide ongoing valid / current auto liability insurance

What You Will Do:

• Call on physicians, area hospitals, skilled nursing facilities, assisted living facilitiesor other senior living settings to promote hospice referrals, Heart to Heart Hospice House,and community resources.
• Working with hospice eligible patients and their loved ones as they begin the process of admission to hospice,in collaboration with the Hospice House staff.
• Develop and implement territory business plans and manages sales time and resources according to plan, adjusting as necessary in order to satisfactorily complete the plan and meet established goals.
• Establish and maintain ongoing public relations efforts to increase brand awareness and community understanding of the organization and its services.
• Utilize customer relationship management software and other tools provided to effectively manage accounts and performance.
