Job summary

Heart to Heart Hospice is seeking a Community Education Representative (Hospice Business Development - Sales) to promote referrals and educate communities about hospice services. The role focuses on building relationships with physicians, hospitals, and senior living facilities, while coordinating with the Hospice House team to guide eligible patients through admission processes.

A strong sales mindset and knowledge of Medicare Hospice Benefit are essential.

Qualifications

• 1-3 years of progressive sales experience in hospice, home health, or healthcare sales preferred.
• Knowledge of business management and Medicare Hospice Benefit.
• Excellent time management and organizational skills; able to meet deadlines and work autonomously.
• Valid driver’s license and reliable transportation with current auto liability insurance.

Responsibilities

• Call on physicians, hospitals, skilled nursing/assisted living facilities to promote hospice referrals and community resources.
• Assist patients and families during admission process in collaboration with Hospice House staff.
• Develop and implement territory business plans and manage sales time/resources to meet goals.
• Establish and maintain ongoing public relations to increase brand awareness and understanding of services.
• Utilize CRM software to effectively manage accounts and performance.
• Perform other duties as required and ensure compliance with laws and policies.

What You Must Have:

What You Will Do:

• Call on physicians, area hospitals, skilled nursing facilities, assisted living facilities or other senior living settings to promote hospice referrals, Heart to Heart Hospice House, and community resources.
• Working with hospice eligible patients and their loved ones as they begin the process of admission to hospice, in collaboration with the Hospice House staff.
• Develop and implement territory business plans and manages sales time and resources according to plan, adjusting as necessary in order to satisfactorily complete the plan and meet established goals.
• Establish and maintain ongoing public relations efforts to increase brand awareness and community understanding of the organization and its services.
• Utilize customer relationship management software and other tools provided to effectively manage accounts and performance.
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
