Job summary

Heart to Heart Hospice is seeking a Community Education Representative (Hospice Business Development - Sales) to drive physician and facility referrals and expand community engagement. You will develop territory plans, promote hospice services, and collaborate with the Hospice House team to support admission processes for eligible patients.

Qualifications

• 1–3 years of progressive hospice/home health or related healthcare sales experience preferred
• Knowledge of business management and Medicare Hospice Benefit
• Excellent time management and organizational skills with the ability to meet deadlines and work autonomously
• Valid driver’s license and reliable transportation with ongoing auto liability insurance

Responsibilities

• Call on physicians, area hospitals, skilled nursing facilities, assisted living facilities or other senior living settings to promote hospice referrals and Heart to Heart Hospice House
• Collaborate with Hospice House staff to admit hospice-eligible patients and their loved ones
• Develop and implement territory business plans and manage sales time and resources to meet goals
• Establish and maintain public relations to increase brand awareness and community understanding of services
• Utilize CRM and other tools to manage accounts and performance
• Perform other duties as required and ensure compliance with laws and policies

What You Must Have:

• One to three years of progressive sales experience in hospice sales, home health, or other healthcare sales strongly preferred.
• Knowledge of business management and a strong knowledge of the Medicare Hospice Benefit

What You Will Do:

• Call on physicians, area hospitals, skilled nursing facilities, assisted living facilitiesor other senior living settings to promote hospice referrals, Heart to Heart Hospice House,and community resources.
• Working with hospice eligible patients and their loved ones as they begin the process of admission to hospice,in collaboration with the Hospice House staff.
• Develop and implement territory business plans and manages sales time and resources according to plan, adjusting as necessary in order to satisfactorily complete the plan and meet established goals.
• Establish and maintain ongoing public relations efforts to increase brand awareness and community understanding of the organization and its services.
• Utilize customer relationship management software and other tools provided to effectively manage accounts and performance.
• Performance of other duties as required
