Job summary

Heart to Heart Hospice seeks a Community Education Representative to drive hospice referrals by engaging physicians, hospitals and senior care facilities. You will develop territory plans, manage time and resources, and build strong relationships to expand awareness of our services.

You will work with hospice-eligible patients and families during admission, coordinate with the Hospice House staff, and use CRM tools to track accounts and performance while ensuring regulatory compliance and

Qualifications

• 1–3 years of progressive sales experience in hospice, home health, or healthcare sales.
• Knowledge of the Medicare Hospice Benefit.
• Excellent time management and organizational skills.
• Valid driver’s license and reliable transportation with current auto liability insurance.

Responsibilities

• Call on physicians, hospitals, SNFs, ALFs and other senior settings to promote referrals and hospice services.
• Collaborate with Hospice House staff to admit eligible patients.
• Develop and implement territory business plans and manage sales time/resources.
• Establish and maintain public relations to raise brand awareness and community understanding.
• Utilize CRM software and tools to manage accounts and measure performance.
• Perform other duties as required and ensure compliance with laws and policies.

Community Education Representative (Hospice Business Development - Sales)

What You Must Have:

• One to three years of progressive sales experience in hospice sales, home health, or other healthcare sales strongly preferred.
• Knowledge of business management and a strong knowledge of the Medicare Hospice Benefit
• Excellent time management and organizational skills, with the ability to meet deadlines and work autonomously
• Valid driver’s license
• Reliable transportation and provide ongoing valid / current auto liability insurance

What You Will Do:

• Call on physicians, area hospitals, skilled nursing facilities, assisted living facilities or other senior living settings to promote hospice referrals, Heart to Heart Hospice House, and community resources.
• Working with hospice eligible patients and their loved ones as they begin the process of admission to hospice, in collaboration with the Hospice House staff.
• Develop and implement territory business plans and manages sales time and resources according to plan, adjusting as necessary in order to satisfactorily complete the plan and meet established goals.
• Establish and maintain ongoing public relations efforts to increase brand awareness and community understanding of the organization and its services.
• Utilize customer relationship management software and other tools provided to effectively manage accounts and performance.
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
