Job summary

Sertoma Star Services in Northwest Indiana is seeking a Direct Support Professional (DSP) to provide person-centered support to individuals with intellectual/developmental disabilities in community-based settings.

You will work closely with consumers, families, and staff to promote independence, health, and safety, while documenting progress and coordinating with care teams. A High School Diploma is preferred; valid driver's license required; full- and part-time schedules available in Indiana.

Qualifications

• Good verbal and written communication skills.
• Ability to work with coworkers and consumers.
• Knowledge of working with individuals with disabilities.
• Ability to work independently and follow instructions.
• High School Diploma preferred.
• Valid driver's license and driving eligibility when required.

Responsibilities

• Provide person centered supports to individuals in community settings.
• Assist with daily living tasks to promote independence and safety.
• Support meal planning and preparation per plans and safety rules.
• Assist with medication administration per agency procedures.
• Document progress notes, MARs, incidents and injuries timely.
• Collaborate with staff, providers and family members for quality service.
