Job summary

Sertoma Star Services is hiring a Direct Support Professional for its Community Day Services in Chicago Heights. You will support individuals with disabilities and mental health challenges to build skills, foster independence, and participate in daily life and the broader community.

As a DSP, you will work closely with consumers and families, document progress, and cooperate with staff to ensure high-quality services.

Qualifications

• High school diploma or GED required.
• Valid drivers license and ability to meet driving requirements.
• PACE certification within 3 months of hire; maintain while in CDS program.

Responsibilities

• Communicate with consumers using preferred communication methods to assess likes, needs, and goals.
• Treat consumers with dignity and respect while promoting independence.
• Assist consumers to achieve Personal Plan/Implementation Strategy outcomes.
• Provide training by doing tasks with the consumer, not for them.
• Support consumers in community participation and natural supports development.
• Advocate for consumer wants and rights as needed.
• Ensure timely documentation of supports provided (progress notes, incidents, attendance).
• Transport consumers to CDS programs and community activities.
• Help consumers learn job skills on the production line.
