• Develops written client service plans with goals to be achieved.
• Provides advocacy and case management to individuals and families.
• Collaborates with community resources to connect clients to services.
• Works within contract, grant, and program requirements including timelines and reporting.
• Manages crisis situations with supervisory approval.
